  Documents Required for Fresh Passport

Two Copies of the following Documents is required:

  12 Passport size Photographs with white background (size)

  School leaving certificate or birth certificate (Original + 2 photocopies with self-attested).

  Educational qualification certificate (12th Standard or Any degree certificate in Original + 2 photocopies).

  Marriage certificate (only for female applicant, incase, if married).

  For residence address proof, any 2 documents required such as ration card, electricity bill, telephone bill, municipal tax bill, bank account passbook or election card.

  NOC from head of the department for  employee of government and semi  government (With given prescribed format).

  For Minor applicant you need to submit passport photocopies of parents. In case you are not holding any passport you need to submit minor affidavit.

  Applicable Passport fee

  Adult applicant: Rs.1,000/- + Service Charge

  Minor (age upto 15 years) applicant: Rs.600/- + Service Charge

  Damage / Lost Passport: Rs.2500/- + Service Charge

The TATKAAL fee is in addition to the applicable passport fee and payable either in cash or DD in favor of Passport Officer concerned. The additional fee for out of turn TATKAAL passport
